Worksite Forms

Worksites who would like to apply to participate in the Summer Youth Employment Program will need to complete a Worksite Application and submit it fax or mail.

Please submit a completed worksite application along with the completed job description attachment by April 15, 2010. The job description attachment must be completed for each position at the worksite. Please make sure to include your desired dress code, a list of tasks you expect the youth to perform, the hours of work required each week and any pre-employment requirements of your worksite (TB test, immunization boosters, special training, etc.).
